Insurance Claim – Auto Injury & Workers Comp

When we treat a patient of record due to an injury caused by a car accident or work accident the form below must be filled out and submitted to the insurance team. The insurance team will then submit the claim to the proper place and follow up to make sure it is paid and closed.
